Football Emblem

Jason Andrew Hoffmann was born on June 5, 1996, to John and Karen (Johnson) Hoffmann in Denison, Iowa. He was raised in Dunlap, Iowa. Jason was a sophomore at Boyer Valley High School where he participated in football, basketball and track. He was named to first team all district and was an honorable mention to the Western Valley Conference for football. Jason enjoyed tractor pulling, riding four wheelers and dirt bikes. He was talented with his hands; making, building, or fixing things. Jason had a kind heart and infectious smile. He loved helping people, making them laugh and giving hugs. He was a member of the St. Patrick's Catholic Church.

Jason died on Sunday, December 30, 2012, at the age of 16 years, six months, and 25 days at the Madonna Rehabilitation Hospital in Lincoln, Nebraska following a truck/train accident on October 4, 2012.

Jason is survived by his parents, John and Karen Hoffmann and two brothers, Justin and Tyler Hoffmann all of Dunlap, Iowa; grandparents, Jim and Jeanne Hoffmann of Dunlap, Iowa, Eugene and Carolyn Johnson of Carroll, Iowa; and many aunts, uncles, cousins, and other relatives and friends.
